PLEASE READ THE FOLLOWING INFORMATION PRIOR TO BOOKING IN YOUR NUTRITION CONSULTATIONS.
IT IS A CONDITION FOR YOU TO ACKNOWLEDGE AND AGREE TO THE TERMS AND CONDITIONS.
This document functions as a contract between you, the client and the service provider Elizabeth Pattalis. When purchasing services you acknowledge that you have fully read, understood and agreed to these terms and conditions. Please seek clarification on any content that is not fully understood.
Fee Schedule and Health Fund Claiming
Consultations fees are as stated on my website’s Services page and are current at the time of booking.
The initial consultation consists of two parts. The payment is due and payable at the first consultation. If you fail to comply you will be charged a late fee.
Payment methods:
- Five Dock: Cash, Visa, MasterCard and electronic funds transfer if you choose to organise prior to the consultation.
- Gladesville: Cash, Visa, Mastercard and direct debit.
- Mortlake: Cash, Visa, Mastercard and electronic funds transfer if you choose to organise prior to the consultation.
There is no HICAPS. “Nutrition” is covered by some healthcare insurance providers. Please check your coverage as each provider is different and I cannot know if you are eligible to claim. Health funds have unique codes for claiming if eligible, this will be on your invoice and issued after the appointment.
Appointments
Appointments can be made via phone or email. You will receive a booking confirmation via email at the time of the booking and an appointment reminder two days before via email.
It is your responsibility to make follow up appointments. When purchasing packages:
- weight loss package – all appointments must be used within a 12 week period from your first consult date.
Lab and Pathology Tests
Results are usually sent through to me 1-2 weeks after the lab has received your results. Once received Elizabeth will evaluate them and then contact you to schedule an appointment to discuss the findings. Lab and pathology testing occurs additional costs (provided to you prior to testing) and is generally not covered by Health Funds (you may want to check with your health fund provider).
Lateness
If you are late to your appointment Elizabeth will still stick to the allocated time slot and not run over time, regardless of if she has another appointment booked after you. At her discretion, she may cancel the session if your lateness is great and she feels the service to be provided has been greatly compromised. She cannot provide you with high-quality professional care without sufficient time. If she cancels the session, the cancellation policy is that you will be charged the full fee amount.
Rescheduling by Practitioner
If Elizabeth needs to reschedule your appointment, she will do her best to provide as much notice as reasonably possible and offer you an alternative appointment time.
Rescheduling by Patient
Elizabeth understands that sometimes you may need to re-schedule or even cancel an appointment. Please let her know as soon as possible as this frees up your time slot for others. You can reschedule via email up to 24 hours before your appointment time. If you do not provide at least 24 hours notice my cancellation and refund policy applies.
Cancellations and Refunds
Elizabeth values her time and appreciates it when others do too. Last minute cancellations or no-shows often means that time is unused and wasted. Appointments are limited and at certain times in high demand and she would like to offer these available appointments to clients on waiting lists.
If you are unable to keep your scheduled appointment you must cancel your appointment at least 24 hours beforehand. If you fail to do so you will be charged a fee. For cancellations providing 12 – 24 hours notice you will be charged 50% of your total consultation fee. For cancellations with less than 12 hours notice or that are a no show, you will be charged the full fee cost. Cancelation fees will be emailed and are due within 7 days via EFT.
There are no refunds for any upfront costs you may have made.
